Health Assessment Information
Periodic health assessments are required of all students in the Norwalk Public Schools.
Findings must be recorded on the State of Connecticut Department of Education Health Assessment Record. Student health assessments are required upon entry to the NPS system, preschool, kindergarten, and during grades 6 and 9. In addition, students participating in interscholastic sports must have a health assessment annually.

If you have questions regarding mandatory health assessments, please call your school nurse.
New Students
Students new to Norwalk Public Schools, preschoolers and kindergarteners must have a completed health assessment (including immunization compliance, current hemoglobin and TB risk assessment) on file in the school nurse's office before they may attend school. There is no grace period.

Students entering grades 7 & 10
Grade 6 and grade 9 health assessments must be performed after July 1st of the year in which the child will be entering the respective grade and submitted to the school nurse before the end of that school year. No child will be permitted to enter grade 7 or grade 10 unless this mandate has been met.
