Norwalk Public Schools works with two health care centers to provide school-based health center services in multiple schools across the district.
Dr. Robert E. Appleby School Based Health Centers
The Dr. Robert E. Appleby School Based Health Centers, a program of the Human Services Council, are onsite health center for students at Norwalk High School, Brien McMahon High School, Nathan Hale Middle School, Ponus Ridge STEAM Academy and West Rocks Middle School.
Your child can take advantage of their physical and/or mental health services. The goal of the Dr. Robert E. Appleby School Based Health Centers is to keep students healthy, in the classroom, and ready to learn.
SBHC Services offered:
- Diagnosis/Treatment of Acute and Chronic Illnesses
- Comprehensive Physicals
- Immunizations
- Health Screenings
- Basic Laboratory Tests
- Health Education & Health Counseling
- Individual, Group and Family Counseling
- Crisis Intervention
- Support with School & Life Transitions; Social Skills; and Emotional Health Issues

NCHC School-Based Health & Wellness Center
Norwalk Community Health Center is operating three School-Based Health & Wellness Centers across Norwalk Public Schools including clinics on the campuses of Ponus Ridge STEAM Academy/Concord Magnet School, Brien McMahon High School/Center for Global Studies and the NPS Family Center/Norwalk Next Steps. See below for services that are provided.
To access and complete the consent form for your child to use the School Based Health & Wellness Center, log into the PowerSchool Parent Portal and click on "Forms."
SBHC services offered:
• Diagnosis and treatment of minor illnesses/injuries
• Physicals (sports, school, or pre-employment)
• Health screenings
• Immunizations
• Health education/risk reduction
• Anti-Bullying Informational Services
• Counseling (individual, family, and group)
• Chronic Disease Management (such as Asthma, Eczema) if NCHC is your Primary Care Provider
• Crisis intervention and suicide prevention
• Nutrition/weight management
• Laboratory testing
• Prescribing and dispensing of medications
• Referrals to outside offices as needed
