NPS Vaccination Clinics for Students 12+ to Launch

As you know, we have been working with various providers to schedule vaccination clinics for students 12 years of age and older in Norwalk. We’re pleased to announce that Griffin Hospital and the Department of Public Health will have NPS clinics starting this Friday!

Please see information below regarding the additional dedicated clinics for Norwalk students.

All clinics will offer the Pfizer vaccine, which is the vaccine currently approved for 12+

Please note: Griffin Hospital Clinics require a different consent form than our currently running Norwalk Community Health Center Clinics do. Please see below on how to obtain a Griffin Hospital Consent form for the newly added clinics:

Friday, May 21, Nathan Hale Middle School, 1:00-7:00pm

(Second dose administered June 11 for the Nathan Hale clinic)

Tuesday, May 25, Brien McMahon High School, 1:00-7:00pm

(Second dose administered June 15 for the Brien McMahon clinic)

Tuesday, May 25, Norwalk High School, 1:00-7:00pm

(Second dose administered June 15 for the Norwalk High clinic)

*Eligible students from other schools may attend any of the clinics after school hours

GUARDIAN CONSENT FORMS EXCLUSIVELY FOR GRIFFIN HOSPITAL CLINICS

Any individual under 18 must have a Griffin Hospital guardian consent form completed prior to receiving the vaccination during school hours. Please submit your student’s signed consent form to your school nurse as soon as possible. Completed consent forms must be submitted and verified by the student’s school nurse prior to vaccination during school hours.

There are three ways a parent/guardian can obtain a Griffin Hospital consent form:

  1. Follow this link to print out the Griffin Hospital consent form and submit to your school nurse as soon as possible.
  2. Printed consent forms are available in the school nurse’s office for pick up.
  3. Parents/guardians may also obtain and fill out a consent form on-site during the vaccination clinic.

STUDENT WALK INS WELCOME AFTERSCHOOL HOURS WITH PARENT/GUARDIAN PRESENT

Even if an eligible student (12-18) does not attend the school that the clinic is being held on that day, the eligible student may attend the Griffin Hospital clinic after school hours (up to 6:30pm) to receive their vaccine with a parent/guardian present.

As the school year comes to a close, we are pleased that more students are now eligible to be vaccinated.
